If you are a Community Stakeholder, please follow the instructions below:

1. Open the following link in a new tab:  https://www.accreditationnow.com/
2. Select the “Surveys” tab
3. Select “Community Stakeholder Survey”
4. Type the following into the “Login” section:  10061279
5. Leave the password section blank, you may get a prompt saying “The password must not be blank”, but his can be disregarded. Simply click “Ok” to continue to the survey.
6. From here you can fill out the survey and press “Submit” at the bottom of the page.  All surveys are anonymous.
