Client Survey

If you are a current/former client of Mesilla Valley Community of Hope, please follow the instructions below:

 

  1. Open the following link in a new tab:  https://www.accreditationnow.com/
  2. Select the “Surveys” tab
  3. Select “Client Survey” (the top option)
  4. Type the following into the “Login” section:  10061279
  5. Type the following into the “Password” section:  qogunh
  6. Click the blue “Login” button
  7. Select the Survey that you wish to take
  8. Complete the survey and click “Submit” at the bottom of the page (Providing name information is OPTIONAL)
